- Annual Field Trip/Medical Form-signature(s)
- School-parent student compact-(Three {3} signatures)
- Media release form-signature(s) required
- Volunteer questionnaire-if applicable
- Parent commitment contract-signature required
- After school
- Student handbook-signature required
Return all the completed forms and the registration fee to the school office. Registration will not be processed without full payment of the registration fee and proper completion of all forms.
